Insurance & Payment:

We strive to make fees, insurance participation, and payment expectations as clear as possible before services begin. Unity Neurobehavioral Health participates with select in-network insurance plans and also works with clients who have out-of-network benefits. Because coverage varies substantially by plan, employer, service type, and medical-necessity requirements, insurance benefits are reviewed on an individual basis.

In-Network Insurance

We currently participate with select Aetna and Optum-administered commercial insurance plans.

Certain other commercial plans administered through the Optum/UnitedHealthcare network

Not every plan offered by these insurance companies is included in our network participation. Eligibility may vary based on the specific policy, employer group, geographic region, and behavioral-health administrator.

We encourage prospective clients to provide their insurance information before scheduling so that we can review eligibility and available behavioral-health benefits.

Notice for Insurance-Based Psychological & Neuropsychological Evaluations

Prior authorization may be required before testing begins, and many insurance plans require documentation establishing the clinical and medical necessity of the evaluation. Depending on the plan and reason for referral, a physician or other qualified healthcare provider may also need to provide a referral or order.

In some cases, an initial diagnostic consultation may be necessary before authorization can be requested so that the presenting concerns, relevant history, and clinical necessity for testing can be appropriately evaluated and documented.

Insurance companies determine coverage based on factors that may include the reason for referral, diagnosis, requested testing services, medical-necessity criteria, plan exclusions, and authorization requirements. Certain evaluations, including some educational, occupational, legal, disability, forensic, or other non-medically necessary assessments, may not be covered by health insurance.

Whenever insurance will be used, our practice will review available benefit information and applicable authorization requirements before proceeding with the evaluation. Final coverage and payment determinations remain with the insurance carrier.
